To become a member of the parish, you must be properly registered. Please visit the Parish Office to fill out the registration form and return it to the parish office, or put it in the offering plate at Mass. Additional copies may be obtained from the Parish Office, from the Gift Shop in the narthex of the Church, or from an usher. Please notify the parish office if there are changes to this information in the future.

On the reverse of the Parish Registration Form is a questionnaire to indicate your interest in our many ministries and organizations. We would also like to know if you have any previous ministry or volunteer experience. Please take a moment to check those ministries and organizations that interest you and your family. You may contact them directly using the names and phone numbers provided in this book or you may return this form to the parish office and we will contact the appropriate ministry and have them call you.
